Since September 2021 I had been suffering with palpitations, panic attacks etc and after numerous calls to my Gp was prescribed sertraline. I also had a 24 hour ecg which showed short run of atrial ectopic beats/Svt. I was reviewed by cardiology in December over the phone and discharged as menopausal symptoms. Last week I was out jogging and had an episode of extremely fast heart rate for (approximately 90 seconds according to my Apple Watch) which caused me to faint. I went to RPH A&E and after a stay of 10 hours was told that I was free to go home and an urgent re-referral back to cardiology would be sent. It was only after speaking to my Gp that it transpired that a neurology referral had been done, with no mention of cardiology in my discharge letter sent to my gp. I was given no advice regarding a neurology referral whilst in A&E nor that I shouldn’t drive whilst this is outstanding. Quite an important oversight. I rang the A&E team who also advised that there was no mention of cardiology in my notes, despite my advice to them of the very recent referral and discharge from cardiology. The neurology referral still hasn’t been reviewed 10 days later, but a re-referral from my Gp back to cardiology has been set up and appointment booked ( for 12 weeks time!) the day after she made the referral. Whilst I understand it may be useful to rule out any neurological cause, given my recent symptomatic history regarding palpitations and Svt, I find the failure to accurately record on notes and subsequent miscommunication of relevant information extremely concerning
