"Acute Care at Home"

About: Older People’s Services / Hospital at Home Northern Sector

(as a staff member posting for a carer/relative),

Story told by the wife of a  male patient in his 70's from the Derry/Londonderry area.

Dr Khan and the team arrived at 1.30pm after my husband was seen his GP at 11.50am.  My husband was treated by the team and gave us great support and hope.  The nurses came for the next four days to give IV antibiotic injections and also the Occupational Therapist brought a toilet seat, electric cushion and dressing seat.  We could not have had better support.  The doctor's, nurses and Pharmacist were all very clear about medications.  Thank you.

Thanks you for your kind response. I am delighted to hear of your positive outcome with the team. Acute care at home is a multidisciplinary team and when admitted we will treat the patient in an holistic approach not just treat the illness. The pharmacist and OT are an essential part of the team and I am delighted that they were able to provide support and equipment to your husband to improve his quality of life long-term. I will pass on your positive experience to DR Khan and the team.
