"The Urgent Community Care Team"

About: Community Therapy Services / Nottingham West Community Therapy

(as a service user),

Mum was discharged from hospital with Covid and without an adequate care package in place. My sister & I were at our wits end when our GP put us in touch with the Urgent Care Team. Paul the Physio came out to do an urgent assessment that afternoon and was an absolute Angel. He was followed by various other members of the team Ian. Thomasina & Sally who also came out to assess Mum & show us how to use the various pieces of equipment. They were all so professional & empathetic, & made Mums last few days much easier. Not forgetting too Amy who checking in with us every day and Manager Sally Ann who helped us in so many ways to get the correct care for Mum. You were all brilliant.

Thank you so much for taking the time to compliment our staff and service, particularly so soon after your bereavement. I am sorry to hear of your mum's passing, please accept our condolences.

Our urgent care team strives to support everyone who is struggling and wishes to remain at home. Supporting the family is equally as important as supporting the patient and I am pleased you felt we had offered you and your sister this support and empathy.

Thank you for your kind words about the staff involved with your mum. I will be sure to pass on your thoughts regarding the positive contribution and impact our staff made to your mum's last few days. It is always nice to hear that we have enabled a family to remain together in such difficult circumstances. I know those days will not have been easy for you all.

I am so sorry for the loss of your mum. Take care of each other.
