I was diagnosed in February this year with Atrial Fibrillation and the GP referred me to see a cardiologist. My condition worsened and I was admitted to hospital as an emergency before my appointment with a cardiologist.
I was in hospital for 6 days, and was treated very well, as they increased my medication to try and control my heart rhythm. I was then given an Echo Cardiogram and released the same day.
I spoke to a doctor before my release who explained that my AF would be controlled by medication and I would be discharged to the care of the heart failure liaison nurse in the community who would adjust my medication as required. This was also repeated in my discharge letter.
This never happened and I have spent the last 6 months trying to find out who should be monitoring my medication and condition.
I can provide details of the contact I have had with the consultant’s secretary, GP Practice and heart failure liaison nurse.
I don’t raise this issue with you lightly as I am aware of the pressure the NHS has been under during the pandemic, but I feel the follow up care I have received falls below an acceptable standard and should be evaluated to see if there are improvements to be made. I feel I have fallen through cracks in the system and have been left in limbo.
