Acute Care at Home team are a valuable service.  Always pleasant and approachable.

The team provided all the items required from the Pharmacist.

Thankyou for posting on behalf of a service user.

We work very hard to provide a valuable service for Older People.

We have a full Multidisciplinary Team. Each member in our team team strives to provide the best care they can. I will share your post with the staff and speak to our Pharmacist regarding her excellent input to Patient service.
