Text from GP said go to Adult Learning centre, so went to this place as per signs but told it was a one way no entry, enter via main entrance. This is what the text should of said.
Given form to fill out & the vaccine card. Should of been told to keep the card safe as nobody else would need to see this during vaccine procedure.
Why fill out this form when GP should have all this info on each patient. Family members have not had to fill out forms when they had vaccine.
