"Social Prescribing Team"

The Social Prescribing team is a Great service with professional staff who truly care about helping others.

The team helped me through a difficult period and gave me alot of support and assistance.

I was referred to a support group for my long term health condition, given a link to receive food groceries at a reasonable rate to be delivered to my home to assist me in my time of need and I was introduced to a Breathing and Mindfulness support session and reading material to support my needs.

This service is really important as seconadry support system for patient's and I recommend it to anyone who feels they need support with there health.

My GP made a good decision to refer me to this team it really helped me and I've gained so much from it.
