(as a parent/guardian),

The wording under parental responsibility on your consent for health reviews and immunisation school children form should be reviewed. It calls out specifically legally adoptive parents. As a legally adoptive parent I am his mother - there is no confusion about this. This line in your form is offensive. Birth mother is nowhere on the form so I’m not sure why you’re calling out adoptive parents separately - it is frustrating.
