Application to register permanently with a General Medical Practice

Acting on behalf of an elderly man who returned after 20 months to his home of very many years, I downloaded and completed the above form as a PDF file on my computer. He wished to re register with the same medical practice as he had been with for in excess of 50 years.

The practice also had their own form. I completed this also on computer.

I submitted both typed forms yesterday. The receptionist complained that I had not completed the NHS form. I pointed out that the answers were all contained within the boxes provided. This receptionist said they had never seen a typewritten form before.

Today I received a message from the acting practice manager. I was told that typed forms were not acceptable to the NHS. I have a disability caused by lack of dexterity and always complete forms on computer.

It is utterly inconceivable that any form available to be completed online must be completed by hand. It would also be unlawful under the Equality Act.

I am sorry to read of the issues you are experiencing whilst trying to register with a GP practice.
