"Gratitude for support in the community"

We are so grateful to have had a supportive and caring team around us when our Mum / Nana, was being cared for at home [by multiple teams].

Single Point of Access at Harrow St Luke’s, Angela, was always calm and professional with a caring attitude and always contacted us back to let us know what was happening if she had to send someone else. This communication was particularly important and it restored our faith in health professionals.

Community palliative care nurse Trudi. Trudi was the first person who visited our mum to support and advise her and the family with any concerns and questions we may have had about her diagnosis, care and treatment. She was calm and caring and always available to answer any queries that we had. When we found it difficult to get hold of the doctors, Trudi assisted by also contacting them, which took a lot of the stress away from us. When we had difficulties or complications, Trudi was the first person our mum's eldest daughter thought of to contact as she trusted that Trudi would do whatever she could to assist.

Janine, Mimi and team from RMH [care agency] were exceptional. With their kindness and caring attitude and respect, this made having her at home much easier than it could have been. Janine went over and above her duty by coming out the evening that our mum came home as they had not been informed by the hospital, we would recommend them as carers for any family in the same circumstances.

We are so grateful.  Your input was invaluable to us at such a distressing time.

We are glad to read that you found the support and communication from multiple community teams invaluable when caring for your Mum at home. We recognise the importance of effective and supportive communication with patients and families at what is for many people, including yourselves, such a challenging and upsetting time. A key role of the Single Point of Access service is to provide patients and families with a single contact number and to help coordinate care across multiple agencies to ensure that the best care is provided.

Single Point of Access works alongside with Hospice at home, community palliative nurses and domiciliary agencies. Our goal is to collaborate and provide quality of care.

It is lovely to hear that staff went above and beyond for you all. Thank you for your kind words and these have been passed on to colleagues at the hospice.
