"Failings on discharge"

About: Queen Elizabeth University Hospital Glasgow / General medicine (Wards 5d)

(as a carer),

My Dad was discharged last night from Ward 6B very unsteady on his feet and delirious - I would have liked to see the results of the 4AT test before departure - if I had seen him in the ward I wouldn’t have brought him home! He arrived in reception unsteady on his feet and very confused and emotional. My sister had to stay overnight and I relieved her this morning - now waiting on emergency GP coming to see him as his head pain is intense and he is in a very confused state.
