"Rde discharge"

About: Royal Devon University Healthcare NHS Foundation Trust

My father in his 60s was discharged on Saturday evening.

The ward did not contact me to collect him, nor did they allow him to call me or a taxi.

He has chronic COPD, arthritis and muscle weakness.

He left in the rain with no phone, money, coat or inhaler in a city he does not know 15miles from home.

He walked over a mile before he found a taxi.

They had a duty of care to contact me as his care person.

I thought he was in hospital safe instead he was walking city streets.

Then I had to pay £40 for his taxi instead of £5 fuel out of universal credit.

You need to make sure patients are safe when leaving
