"Complete disaster"

My father was transferred from Queens Hospital over night to the respiratory unit. He was there for a short period and then transferred to the Southwell Ward. My dad was deterred from trying to use the phone to call me, I was given incorrect information about my father's discharge and the time he was meeting with the Dr. My father was trying to give the nurses a card that we had made up for him as he has had a stroke and has APHASIA, meaning the absence of words. No one except one nurse took the time to read the card, they assumed my father had dementia and what I understand is that he was put on a ward with other patients that had dementia too. When the one attentive nurse took the time to read the card, she realized that my father was in the wrong place and that he has APHASIA, not DEMENTIA. My father lives independently at home, in British Columbia Canada, and also has a home in the UK. When I brought my father home, the information on the Discharge Summary was also incorrect, showing that my father has Expressive Dysphagia..this is not accurate, as DYSPHAGIA is difficulty swallowing, APHASIA is what my father has, the absence of words. Also the DR information is incorrect as well. Definitely the worst hospital experience for my father to endure, and we will be following up with a written report to the Director of the hospital.

I am sorry to hear about your fathers experience on Southwell Ward in particular the breakdown in communication. This must have been very frustrating for yourself and your father.
