"Lack of communication to staff"

About: Derriford Hospital / Respiratory medicine

my mother was admitted to hexworthy ward, ( non respiratory) due to the lack of beds elsewhere. We experienced 4 injuries/ incidents in 4 days. I would like to stress this was due to inadequate staff levels and non communication between shifts. We kept stressing how important it was for each patient to be assessed individually. As each patient had different needs/ requirements. It was a horrendous experience for my mother and ourselves and only stating I would raise it as safeguarding did I feel I'd made a breakthrough. I think the trust need to consider how much pressure the staff are under, they can only operate effectively if there are enough of them to manage the ward safely. Once the staff knew how to treat my mother did things improve but it took far longer and with intervention from other departments for this to occur. The doctors and staff are under enormous pressure but I cannot state enough that each patient have different needs and requirements and this needs to be addressed ASAP. Having spent a total of almost 72 hours with my mother over 8 days ( not counting other family members after I left) this seriously cannot continue. As the staff are in fact very good but have far too much to contend with.

I am very sorry to hear of your concerns.

I was aware of the issues you have raised, especially in respect of the staffing levels on the ward.

I am also conscious that your own experience of community nursing is very different to that within a hospital that manages wards with a high acuity, and where there are many very sick patients with advanced care needs.

I am pleased to record that when your family visited the ward you discussed your mother's care with the medical team at each request to do so.

I can assure you that when you raised concerns, they were each addressed immediately and I recall my staff did everything they possibly could to accommodate your requests and to answer your concerns.

Please be assured that the team on Hexworthy ward strive to deliver person centred care to every patient. As mentioned above, the ward is very busy as a patient's needs are difficult to predict.

However, staffing levels are adjusted accordingly and in the event of unpredicted sickness these levels will naturally fluctuate.

In addition, communication is paramount to the care pathway of every patient and every effort is made to document and handover vital information at all times.

I hope this provides you with the reassurance you need at what was undoubtedly a very difficult time.
