"the help i received from the Hospital to Home team at BGH"

About: Borders Community Services / Hospital to home service

(as a service user),

I had been in the BGH for a few days with a weakness in my right leg due to nerve damage and could walk only with a zimmer or 2 sticks .Although I live alone my daugter who lives 200 miles away was with me for a few days when the phone rang . It was David from the team at BGH asking if I needed any help. That was the best thing that had happened to me since getting home ! David came and was very helpful and encouraging. He gave me lots if exercises to do plus lots of good advice . He came back with various walking aids, showed me how to use them and every day from then on someone from the team came to accompany me when I was practising walking. That was very good because I had no confidence going out on my own . David appeared from time to time to assess my progress until he decided that I no longer needed him or the other carers.

Thanks to them I can now manage on my own.I cannot praise enough

Thank you for your feedback regarding NHS Borders new Hospital to Home Service. We greatly appreciate the time you have taken to share this with us. I am so pleased you have benefited greatly form our Physio David and Health Care Support workers input.Our aim is to work closely as a Multi Disciplinary Team to re-able individuals to return to their previous functional baseline as quickly and safely as possible
