"No careplan in place"

About: Forth Valley Royal Hospital / Stroke care Outpatients Centre / Outreach clinics

(as a carer),

My mother suffered a stroke lost control in right side, recieved major op and released 4 days later from forth valley royal infirmary with no careplan in place, promised outreach help for rehab and necessary care. No support and having to wait a further  8-10 weeks before considered for physio. I believed physio was an accentual part of recovery.
