As the partner of a patient who has been under the care of the same doctor for the past seven years,I have become saddened and disillusioned by the lack of a joined up approach to delivery of care .I have nothing but praise for this doctor and his team ...But ...beaurocracy alongside a very narrow view of holistic care by administrators who in my opinion have no real understanding of patient and carer need have made a very difficult time greatly more distressing than need be.The Trust outsources service s to other Trusts .In this particular case my partner had a liver transplant in 2013 at St James hospital Leeds ...Therafter his health requirements were to be delivered between Leeds and Preston .Unfortunately due to the complex nature of care required my partner was left somewhat in limbo ...No coordinated support was considered to the psychological effect upon patient or carer in regard to the huge life changing issues that such surgery brings .I could write a book on our experience .The fact is evident there is a fundamental need for a Nurse/Doctor coordinator service alongside a support group for people who have their care delivered across two or more Trusts .It is imperative for a good recovery for a patient and carer to have a consistent point of contact There needs to be discussion between all parties in order to ensure the best possible outcomes for all.
