"My Father's end of life care"

About: General practices in Lanarkshire Lanarkshire Community Services / Community Nursing Service

(as a carer),

As a service manager in a NHS hosted service I had a working knowledge of how services work together to support care in the community. However I felt humbled to witness the support we received when my Father became frail recently. As a family we decided to support my Father's decision to die at home and we organised ourselves to provide that care. However without the professional and sensitive support which we received from Dr Arnott the GP, the District Nursing Team and the Home Support Carers team, we would have struggled to meet all of my Father's needs at home.

Nothing was too much and each of the services dovetailed to provide support and continuity to his care. On reflection we are glad that we supported his decision but recognise that without the services we received from the local health and care teams we may have reverted to hospital care.

On behalf of my sisters, our partners and  myself, we would like to pass on our heartfelt thanks.

Dear Scooter,

Thank you for taking the time to share you and your family’s experience of caring for your father during his final illness.

I am glad our locality health and care teams were able to support your family to enable your father to be at home as he wished.

The teams work very hard together with patients and families to meet their needs and we appreciate your acknowledgement at what must be a difficult for you.

I will pass on your heartfelt thanks to the teams involved.

Thank you

Morag

Dear Scooter,

Thank you so much for getting in touch with us to give us your positive feedback. If it's alright with you I will share this with the practice manager and Dr Arnott at the GP Practice as they are currently not able to see this. I'm sure they will appreciate your words, particularly at what is a difficult time for your and your family.

All the best.

Laura
