As a service manager in a NHS hosted service I had a working knowledge of how services work together to support care in the community. However I felt humbled to witness the support we received when my Father became frail recently. As a family we decided to support my Father's decision to die at home and we organised ourselves to provide that care. However without the professional and sensitive support which we received from Dr Arnott the GP, the District Nursing Team and the Home Support Carers team, we would have struggled to meet all of my Father's needs at home.
Nothing was too much and each of the services dovetailed to provide support and continuity to his care. On reflection we are glad that we supported his decision but recognise that without the services we received from the local health and care teams we may have reverted to hospital care.
