(as the patient),

I write with reference to Lisa, my Diabetes Specialist Nurse, and would just like to mention how happy I am with the fantastic service she has provided for me.

I have struggled with Diabetes type 2 control and was again referred back to Worcester Diabetes centre to help me take control last year.

Lisa reviewed my medication and started me on a fairly new drug and amended my medication. I have now lost over 2 stone in weight (now just over 13 stone) My weight, again something I have struggled with as well, as you can see has improved and my hba1c has dropped considerably, and no more hypo's and hyper's too!

Thank you Lisa and the team for all your help and support! Life is so much better! You have made such a difference!

Please can you ensure the Matron is aware of her fantastic work for me please.
