I feel it would be good practice for staff to introduce themselves when letting visitors into maple ward stating there name and job title because I had know idea who anyone was when visiting my partner recently, it would be very helpful to identify staff. Only one person did this the whole two weeks I was visiting, a nurse.

Also I was not informed of my partners discharge or given any discharge information,(we live together). My partner was given some information prior to his discharge but being anxious about coming home could not remembered all information given to him, again poor communication between clients and staff which is not good practice.

I hope these comments will be taken on board to improve patient care.

I am really sorry to hear of your experience. As a ward we strive to give the best care possible and the way we can do this is by building relationships with both service users and their family and carers. I personally apologise that you felt that staff did not introduce themselves and or identify their roles, which I can appreciate can cause confusion. I will reiterate your suggestion for staff to introduce themselves in our next team meeting.

With regards to the lack of communication prior to your partner being discharged, again please accept my sincere apology if this was the case, as this goes against our discharge planning process. Generally we always discuss and give patients information about discharge. This information usually includes support post discharge- which team will support, medication that will be given and any other issues that may have been identified during the admission. This again is often communicated with the family and significant others. I have taken on board your concerns and will discuss this with the rest of the team.

If you wish to discuss this further I will be happy meet with you to discuss how we can improve communication about discharge information.
