"Poor administration and inadequate family..."

Regarding admin: various issues throughout 2017 - awaiting results from a private clinic to be passed on to me. Ended up having to get the private clinic to send it direct to me. 2018 I also asked for a GP referral letter and was told that it was done and that I could come and collect it. When I arrive to collect it I am then told to return tomorrow. They say it’s not been printed out and they cannot print it out. I specifically checked it would be ready. I had to insist on waiting for the letter and was then told they would print it out for me, as if they were doing me a massive favour.

I recently asked for an appointment with a practive nurse to discuss contraception. I was told the practice nurses do not do this? I was then asked several questions about what I wanted to know about contraception by the receptionist, which I think is inappropriate. Then I was told they needed to book me in for a telephone consultation with a doctor in a months time. This would not be any help if I wanted to be measured for a cap. I went instead to crown heights. They would at least see me. Then I called back the GP to ask for a coil insertion appointment. I was told I would have to wait a month to speak to the doctor on the phone who may/May not then fit it at another appointment. I think this is a very poor service and this type of neglect of family planning and sexual health provision will have consequences for some people.

Thank you for your comments. As I have responded previously, we are always happy to meet with you and go through your concerns in detail, but I will respond in general terms as you have chosen to post here.

Like all of the NHS, we work under tremendous pressure. I am currently on leave, but am responding today as it is unlikely I will have time to respond on my return on Monday, such are the competing demands on our time.

We have prioritised those who are acutely ill or in need of urgent medical attention, and it necessarily follows that planned procedures such as minor surgery or coil fitting have to wait a little longer than they once did. We offer a full range of family planning including emergency contraception at short notice by our same day doctor. We offer telephone consultation to discuss options and concerns. These are usually in 2-3 weeks and by asking for details, the receptionist can book you in with an appropriate clinician. Our practice nurses do routine “pill checks” but do not fit caps or coils. Some GPs offer specialist family planning advice.

Once we are sure the coil is the correct choice for you, we offer an appointment to fit in with the timing of your period. As with other appointments, it is unfortunate that a fair proportion of these appointments are wasted as patients neither cancel nor arrive. This wastes time with both the GP and the nurse and means everyone else has to wait longer.

Regarding results from Specialists and referral letters, we encourage patients to register for online access to their records, giving immediate access to both of these aspects. We encourage patients to ask their specialist to copy them in to all clinical letters from the hospital and to ask the specialist for any sick note they may require, reducing delays, avoiding confusion and giving us more time to help other patients.

Finally, we use eConsults, we are now getting 20-30 a day, to allow rapid responses where there is concern or confusion or simple requests eg a copy of a letter. We could have signposted you to NHS Choices for detailed patient information about contraceptive choices, and that the family planning clinic at Crown Heights is open to all and offers coils and caps, but there may be a wait.
