"Equipment failure"

About: Aberdeen Royal Infirmary / Radiology (X-rays and Scans)

(as the patient),

I was taken into theatre and put to sleep for my embolisation.  When I was woken up I was told that they were unable to do my operation.  I was told that they were in the crucial part of my operation and everything was in place to proceed when the camera which was inside me stopped working.  The machine located in Special procedures 1 Theatre in In-patients Radiology Department was switched off and restarted twice while still inside me.  my Consultant was forced to take the decision to stop the procedure, as it was putting me in great danger.  This was obviously very frustrating for the whole team, as they had all stayed back late to do my operation.  The team were all fantastic with me while waiting to go through to theatre and especially the member of staff who stayed with me in recovery and made sure I got back to my ward even though the fire alarms were going off.  It was late until I got back onto the ward and the next day I found out that the team’s superiors were already aware with a fault with the machine as I believe this wasn’t the first time the machine had stopped working during a procedure.  I have been advised that the machine had not been looked at previously as there was no funding for the machine as it is under 10 years old.  The problem had also fallen on ‘deaf ears’.  This  team do lifesaving procedures with this machine and it is not acceptable for it not to be working, as I understand Aberdeen only has the one machine.

Thank you for your comments and I shall follow this up with the teams involved sharing your concerns and thanks to the clinical team involved.

I hope I can reassure you that the issues you have raised are being listened to and actioned as a matter of urgency.

The equipment in Special Procedure room 1 and Special Procedure room 2 are at the top of our replacement plan for radiology. We apologise for the obvious distress, discomfort and disappointment this incident has resulted in for yourself and the need for the clinical team to manage your situation as a result of the breakdown. We have already had the engineers attend to identify the problems and enact repairs and will fully quality assess the machine prior to being used once again.

Your kind words about the team and individuals will be shared and I am sure they will appreciate your positive thoughts.
