"Poor care following transfer from different hospital"

(as a relative),

My mother was transferred to Swindon from a different hospital post burr hole surgery.

Since this time I feel she has DRAMATICALLY declined due to an infection and testing positive for flu.  I have shared full insights with Doctors countless times, notes are taken never shared with the next doctor.  

Her decline is considerable since discharge from the previous hospital, she was becoming lucid, able to shower herself with minimal assistance and enjoying meals,  now she is constantly hallucinating, wears a diaper, has been soiling herself,  defecates on the floor and if not prompted between mouthfuls does not manage to eat.  

She has been at GWH for several weeks at first I believed  I got traction with the nursing team but it is becoming clear that calling in for an update is pointless as asking if she had a good night’s sleep gets a stock answer and I get a real answer from other patients in the bay.  She does wander on the ward but I never see attempts to orientate her and I think it is very, very evident from her bloods that she is dehydrated which will contribute to her delerium. But the team on the ward don’t seem to have the time or care to hydrate her which seems illogical.  

Finally speaking to the Ward Doctor it has also come to light there was never a full transfer of notes from the previous hospital to Swindon and it seems that no one has thought to ask, instead they rely on what is immediately presented so defining deterioration would be impossible.

In my opinion, this hospital has done nothing to address my mother’s needs which is to hydrate her and orientate her or actually engage with the team who cared for her prior to transfer, if they had they might actually understand the days prior to arriving at the hospital she was not as demented as she is now.  The nursing is really up to the individual nurse she is allocated.  The ward Doctors Present her current state to in house consultants who have no start point to measure against and they make assumptions that are incorrect due to not being given the entire background,  all information of which  I have shared  and the other hospital team would be able to give insight but have not been asked.   

I would conclude that this hospital is not skilled to look after post burr hole surgery patient’s.  The ward doctor told me they were not knowledgeable in this area.  During her time on a neurosurgeon surgery ward at the first hospital my mother was orientated and rehydrated when her blood tests showed the need, she was also allocated a side room to reduce her agitation, Swindon chose to prescribe antipsychotic drugs which have made no impact.  The difference a side room and rehydrating with drip has been shared with ward staff at Swindon.
