"Diabetic Care"

I was diagnosed as a Type 2 diabetic last October and undertook a treatment plan with the in house Diabetic Nurse.

I have found the whole experience to be thoroughly professional, well ordered and importantly highly beneficial. I am pleased to say that I am making good progress under the nurse's care and have acquired a good knowledge of the condition itself and how I can help myself manage it. I find the Care Planning Results Sheet particularly useful as my 'yard stick' and really appreciate being able to view my results online

Finally I am really pleased that I shall be able maintain my contact with the surgery once the merger goes through later this year.

Thank-you so much for taking the time to provide such positive feedback about your diabetic care. We have worked hard over a number of years to improve the care we provide and particularly spending time to ensure patients are able to develop knowledge of their condition and the ability to self-manage as far as possible, supported by our clinical team. I am delighted that you are finding the whole process to be such a positive experience. We are now developing this across other long term conditions.
