NHS trusts have processes and procedures for a reason, to ensure that patient recieved appropriate and safe treatment.
The patients best interests should be at the heart of all care.
Notes should be kept up to date as per latest CQC inspection and signed, not misplaced 3 times in 2 days and no record of next of kin on file whilst a patient is in surgery.
This is a clear IG breach as not only were the patients observations and drug records misplaced so were the nexts of kins personal details.
AMU attached to Ward 12 does not display the names of Sister, Matron or Ward Manager.

Still waiting a call back from the PALs team to furnish us with this information 48 hours later. Although the staff nurse on duty during the day on the 23rd and 24th of Jan 2018, was a shinning light of best practise, caring and compassion,
The disreguard for close relatives anxiexty and mocking this concern by staff where other relatives and visitors can hear, is not acceptable, infact this is not acceptabe full stop where ever the staff are.
It makes a mockery of seeking friends and family feed back.
Walsall Healthcare NHS Trust please reflect seriously on how you wish to deliver care to patients and relatives to.
All issues we experienced have been identified in the 2017 CQC inspection, changng cultrue is never going to easy, please stand back and look in through a patient and relatives eyes.
