About: The Wellspring Surgery

I visited the health center on Friday 9-6-17 request to fill a form for me,it is a medical statement that I need to provide to my job. At the first the receptionist look at me like I am from other planet,it was very hard to explain what I really need them to do for me. After 15 min of my explanation they finally understand and they told me that it will cost 30£ to make it,we start argue a little about why I need to pay this money and about the day that I will receive it.they ask me to fill the form and leave it with them and come to collect it on Tuesday the 13-6-17..I ask them if I can pay now and they told me no you don't need to do it we will make it for free. Next think I did is that I came to take the medical statement today Wednesday 14-6-17 one day after the day that o was told to come. I went to the reception ask for my papers and they told me to wait..

After 30' min they call me in the reception and they give me my paper but they ask me to pay 20£..when I complain about this the receptionist with rood tone they told me to take or leave it and they didn't believe any word of my story...

The most crazy part is that I didn't even see the doctor.. I just tick the boxes and they sign the form and give it back to me..

In the form there wasn't even the name of the doctor or any stamp from gp practice..after my surprise about who is signed this form the receptionist with a different pen -ink- they wrote down the name of the doctor.. This is crazy it is like false signature issue.

This is my story I hope someone will read it and take some action, I could very easy have hepatitis A-B and the doctor didn't even see me...but they signed that I am totally healthy...
