"Lack of information from the social workers"

My mother has been fit to be discharged for a few days but cannot be discharged as there is no care plan in place for her. The care home she was going to while her own home could be assessed said they could not provide the level of care my mother would need, it has taken the hospital four days to tell us this. We were advised we would need to go to a private home which will take a further 3 or 4 days to arrange.

The lack of information from the social workers is making my mother angry and frustrated. She wants to get out of hospital but these delays are making her feel ill, I reckon she has been in hospital for 7 days with nothing wrong wither that a care home could not help with. This would have freed up a bed for someone else.

Dear foxtrotjd76,

I’m sorry to hear of your mother’s frustration and upset whilst waiting to be discharged. We don’t want anyone to stay in hospital longer than is necessary, but it is crucial the right care plan is in place before patients leave hospital.

Discharge planning is an early consideration when someone is admitted to hospital as we know that this can be a complex process and people may need support from multiple services. That said, it is important that we provide clear information and keep our patients and their families up to date on what is happening and I can only apologise that this has not been your experience.
