My care plan is fab. I sit down with my Care Co-ordinator who is a Social Worker and we meet up once a week. He makes sure that time is prioritised to meet up. We regularly review the care plan and make changes if necessary. The Care Plan works for me and I have a lot of input into my care plan. I requested that my parents do not see my care plan and St Andrew's respected that.
