"Enlarged prostrate causing chronic urine retention"

About: Queen Margaret Hospital / Urology Victoria Hospital / Urology

(as the patient),

I was unaware and hadn’t recognised any symptoms until the latter stage when retention caused kidney trauma and the enlarged prostrate had blocked the urethra. I was told that Green Light laser surgery was necessary to erode the prostrate and unblock the urethra.  I was fitted with an in-dwelling catheter.  The staff and level of care at NHS (Scotland) Victoria Hospital in Kirkcaldy was very good and I greatly appreciated their help and support.

The NHS (Scotland) waiting list was long I was told, with no certainty of likely timescale.  The Scottish Government target is 1 year. This was a devastating prospect and so I went private.  Within less than 1 month I had the surgery.  It went very well and I quickly recovered from that. I found the standard of care and quality of the staff very good.  NHS must improve on waiting times for surgical procedures.

Unfortunately I have since been unable to urinate without a catheter and moved on to self use disposable single use catheters which I took to very well. I was recently invited to have a Urinary Dynamic Survey procedure, which concluded my bladder had been stretched so much by the retention problem that it is no longer functioning and unlikely to recover. Permanent self catheterisation will be my new lifestyle.

The Survey procedure was undertaken by the NHS at Victoria Hospital in Kirkcaldy and can’t praise highly enough the assistance, care and attention given by the staff the Urology department.  They are very professional and deliver very high levels of care with attention to detail in maintaining dignity, explaining what is happening and clearly advising on future needs and support. I am now trialling at home the best way and best timescales in which to use the self catheters to ensure I void my bladder before volumes exceed 400ml.  Beyond that I will need to practice good forward planning in terms of catheter supplies and using/travelling away from home.

My advice is to be scrupulous about prostrate, bladder and kidney health and care.  Seek medical help and advice if you notice any changes in urination or wellbeing and have your prostrate checked periodically for size and health.  I think occasional ultrasound checks could have detected my problem earlier.

Dear casingrs76,

Thank you for taking the time to provide feedback on the care received before and after your operation, the waiting times for the GreenLight Laser Prostatectomy, and your advice for others on their Urological health.

It was myself who performed your Urodynamic testing, along with my colleague Terri assisting. Despite the results of this not being what you'd hoped for, I am glad you are taking to the intermitting catheters well. This will take some time for you to find a regime that works for you and your lifestyle. However, should you have any issues with performing this or with the regime etc, please do not hesitate to contact the Urology DTC and we can arrange for a review appointment in the Nurse Led ISC clinic, if needed.

I'm delighted to hear that you felt you were provided with a high level of care and attention during your appointment. Although the test can be daunting and somewhat invasive, we aim for you to be as comfortable as possible with your dignity maintained throughout. We strive for clear communication, in order for you to be fully informed of what to expect during and after the testing.

I am sorry the uncertainty of the surgical NHS waiting times were a devastating prospect to you, for which you then went down a private route. Please be assured there is a lot of work happening behind the scenes to improve these waits for patients.

I wish you all the best going forward and it was my pleasure to be involved in your care.

Gemma - Advanced Clinical Nurse Specialist, Urology DTC.

Dear casingrs76

My name is Jackie and I am the Senior Charge Nurse for the Urology department.

I first of all would like to thank you for telling your story and providing a comprehensive insight into your journey.

I am extremely delighted to hear your high praise for the Urology department and the holistic care and treatment which you received. It is really good to know that you were provided with a very high level of care and attention, and also that you were well informed regarding each of the services which you encountered. As a department we are very passionate about our service and take pride in providing a high level of care.

I am sorry to hear of the lengthy waiting times for the procedure you were recommended, as my colleague has advised there is a lot of work going on in the background to bring us back in line with national government targets. I am however happy to hear that you were able to proceed with this in the private sector.

I would also like to thank you for promoting urinary health and care, hopefully our readers will be inspired by your feedback and seek review if they are experiencing any issues.

I hope you are managing well with the intermittent catheters and have found a regime that works well for you.

I look forward to sharing your feedback with the team.

Best wishes

Jackie
