My husband and I wish to express our heartfelt thanks to numerous healthcare teams within Perth & Kinross Health & Social Care Partnership.
In his 90s and with failing health, we approached P&K social services enquiring if dad would be eligible for some personal assistance at home. Following an initial assessment, it was agreed the Hart Team would assist dad, however in a matter of a couple of weeks, they assessed that dad required additional long term care support which would be provided by Ann Inspire Care.
Over the past 4 months, a number of teams were involved in supporting dad to remain at home in the final journey of his life. These include LINCS (Monica), Perth & Crieff Social Services (Helen, Lucy, Karen & Heather), Equipment Stores Perth (Karen), Occupational Therapy, Comrie Medical Practice including District Nurses, out of hours GP (Dr Cowie), Comrie Pharmacy and lastly Ann Inspire Care (Julia)
This was truly an excellent example of integrated joined up care by health professionals and social services.
We wish to thank all teams and individuals for their support, and compassionate care these past few months which we deeply appreciated.
