Been trying to contact the Breast Screening Clinic all week during the hours stated and using the number(s) given on the letter: 023 9228 3613/4, and the number on the website:023 927 6336, AND via the QA switchboard. Left messages which have not been answered.

As this form can only be submitted by completing all fields I've starred each section 1 and had to put in a fictitious date of a visit,

If you have still been unable to contact them, then please contact the Patient Advice and Liaison Service (PALS) on 0800 917 6039 or by email at PHT.PALS@porthosp.nhs.uk who can raise your concerns with the relevant staff.
