"Poor discharge planning"

About: North West Anglia NHS Foundation Trust

My Father who was discharge on a Friday evening after being in hospital continually for 6 months (on 4 different wards and two hospital)

The discharged failed due the following reasons

-Medication - different details on boxes to the printed sheet

-No Care plan in place - he is 81 and has stage 4 bowel cancer

-No equipment - no OT / SW home assessment was completed to determine what help he required

-No training on how to use the feeding equipment to his local daughter

-No local family support, Dad has 3 girls one lives overseas another 350 miles away and the local daughter works 9 hour days

he lasted 4 days a home

I am so sorry to hear about your fathers experience of being discharged and I am obviously concerned about the comments you have posted. We want to ensure all of our patients receive good quality care and a safe discharge from hospital. I am therefore disappointed that this is not what happened in your fathers case.

Once again my apologies that your fathers discharge was not as we would wish it to have been.
